The Double Barrel Punisher is a NX-Standard Cosmetic Variant of the Double Barrel Shotgun bought in the Black Market. With slightly higher recoil on the listed stats, the Punisher has steel-like colored barrels that are more textured than the regular variant.

- It was available for playing 25 games of One Man Army on the weekend of August 27, 2010.
- On 22th December 2010 11am PST, Nexon retired this weapon from being purchase permanently. It was sold for 7,950 NX for permanent. (However, after the Return of the Perms Sale 2, this weapon, along with almost every other Black Market weapon, had its permanent duration option reintroduced).
- It was available for permanent duration in the Return of the Perms Sale 2 for 15,920 NX.
- The Double Barrel Punisher available from the Myst-Jan/Myst-Mar cases on the EU servers seem to be different than normal; it has 14 shots instead of the normal 22, and counts as a secondary instead of a primary.

- Like the original Double Barrel, the Punisher is subject to failing to discharge both barrels due to lag or low frames per second.
- It's easy to differentiate the Double Barrel Punisher from the Double Barrel. It is also easy to tell them apart from their kill icons, which the Punisher is much larger
